Due to high demand, we ask that new patients complete onboarding paperwork by 5 PM the day before their appointment and attend their initial consultation to establish care. Missed appointments without notice may result in ineligibility for future services.
We do not charge for appointment cancellations. After your 3rd missed appointment, you will no longer be eligible for services.
